1. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2025) and attached Work Experience Sheet (WES) which can be downloaded at www.csc.gov.ph (digitally signed or electronically signed);

2. Hardcopy or electronic copy of Transcript of Records;

3. Hardcopy or electronic copy of proof of eligibility/rating/license;

4. Hardcopy or electronic copy of Performance Rating in the last rating period (if applicable).
